Diabetes Education Assistance:

  • Do you need diabetes education?

    How do I know if I need diabetes education?

    Please answer these brief questions:
    • I have diabetes and have not met with a diabetes educator.
    • I do not have a personal meal plan.
    • I do not have a personal exercise plan.
    • My blood sugars are usually above 130 before meals and 180 two hours after meals.
    • I feel tired and thirsty and have to urinate often.
    • I don’t know how to test my blood sugar.

    If you answered “true” to one or more of these statements you could benefit from diabetes education. Print the Intake Program Prescription form and take it to your doctor to sign and send it to us. This form is necessary for your insurance to cover the cost of education. Your doctor’s signature is required and you may also need an insurance referral.

We can help you deal with the many individual concerns that arise for someone living with diabetes. Your family or significant other is wlecome to be involved. We keep in contact with your doctor about your progress throughout the program. Experienced healthcare experts teach classes year-round.

    Healthcare professionals are welcome to call us about setting up a fast and easy referral system for your patients with customized forms. Our business hours are Monday thru Friday, 8 a.m. – 4:30 p.m. Please contact us at 313-745-9361 to speak with a Diabetes Education Coordinator.

    This form is necessary for your patient’s insurance to cover the cost of education. A doctor’s signature is required and an insurance referral may also be needed.
